Student Leader Onboarding

This form will be used to submit your information to our Shared Service Center to onboard you in your new position. There are additional questions that collect necessary information from all student leaders.

Information in this form is kept confidential with ASUCR professional staff.

The only information that is shared is (1) dietary information or allergies for meetings, events and activities that you might be invited to and (2) Major and Year in college which is included in briefing documents ahead of meetings with campus administrators, UC regents or invited campus guests.

Once submitted, you will receive an e-mail with instructions from Docusign.
In there you will electronically sign forms and make an appointment to visit the Shared Service Center to physically sign forms that need wet signatures. You will be required to bring physical copies of your eligibility to work in the US (Common forms are US Passport or a CA Driver's License AND Social Security card).

PLEASE NOTE - YOU ARE NOT ABLE TO BEGIN WORK UNTIL YOU HAVE COMPLETED THE ABOVE PROCESSES WITH THE SHARED SERVICE CENTER. NO BACK PAY WILL BE PROCESSED.

Failure to complete this form or the appointment with the shared service in a timely fashion can result in consequences including only being paid from the date of completion or a referral to Student Conduct for failing to complete with the directions of a university official. If you have questions about either of these consequences, please connect with the ASUCR Executive Director.
